O efeito da municipalização no sistema hospitalar brasileiroos hospitais de pequeno porte

Resumo

Hospital care accounts for part of the increase in health care costs. Countries have adapted their public policies to the hospital sector, concentrating attention on larger centers, after studies of the 1990s have shows that hospitals with less than 200 beds have reduced efficiency. In 2017 there were 6,787 hospitals in Brazil, 62.3% of them with less than 50 beds. In 2004 the Ministry of Health published the National Policy for Small Hospitals (PNHPP). The present study evaluated the PNHPP and its impact on the Brazilian hospital sector. The results show that adherence to PNHPP occurred in 12 of the 27 States. In the absence of policies to induce the formation of a hospital system as part of a network, privileging the integrality of the actions, the municipalization generated the pulverization of hospital care. This was the way found by the municipal politics to meet health needs. There was an increase in municipal hospital units, with a reduction in size, reaching an average of 50 beds per hospital. The reversal of this scenario involves policies that induce the qualification of hospital care until the understanding that the almost 5,000 small hospital units in the country constitute a broad set to be studied, subdividing it into smaller groups, with different vocations.
