Decentralized planning and ascending the health sector at the municipal levelan experience of the municipal government of Belém

Resumo

A dynamic process, distinct from the one traditionally carried out in the health sector, is the core of the basic strategy the authors intend to present, at municipal level, so that real needs of a community may be incorporated to institutional goals. It is examined here an experience of the municipal government of Belém, where directives were drawn and strategies developed, with a view at reversing the "static" conception of an office-centered planning, traditionally performed by a central technical staff assembled to project sectorial programs, well apart from the communal realities in the area of health services. The adoption of a new direction in the practice of health planning in Belém's municipality, based on principles of sanitary reform, has generated a participative and decentralized managerial process, described in this article in terms of its methodological stages. The study also includes the most significant impacts and results achieved through a more dynamic process of decentralized and ascensional planning, in addition to effecting a critical analysis of institutional circumstances capable either of favouring or hampering the process' development. In other realities of Brazilian public administration, the dynamics of planning may have achieved a higher degree of evolution than the one describe here. The capacity to correct distortions and to improve the practice applied by the Secretaria Municipal de Saúde de Belém (Municipal Secretary of Health of Belém) will, undoubtedly, lead to a better proportioning and allocation of the sector's financial resources, a priority being accorded to what must be done, starting from an ensemble including governmental policies, the epidemiological profile of the municipality and community needs and aspirations.
