A contratualização e a efetividade na assistência à saúde

Resumo

Throughout the years of 2009 to 2015, the transfer of the direct management of the public units of health for nonprofit entities with private rights, qualified as healthcare social organizations (OSS). Such transfer happened with the purpose of improving the quality of the services and rationalize and optimize the usage of public resources. Sharing the management was the path that the State of Rio de Janeiro chose to face the various challenges that were imposed to the public management. Through the management contracts, which formalize the partnerships, contractual goals of production and performance were stablished, in addition to a maximum budget for each one of the units. In this context, the present paper had as a goal evaluating the effectiveness of the contracting process of the State Department of Health (SES/RJ) in the case of the Emergency Care Units (UPA 24H). For this, ‘effectiveness’ has been defined with regard to three perspectives: politics, healthcare and budget. From this point, the literature review was performed in order to allow the construction of the theoretical justification that settles the paradigms of the partnerships by the public sphere, identifying subsequent benefits and problems, in addition to the research and analysis of the dada produced by the SES/RJ. Ultimately, with connection to the political aspect, a conclusion was reached that the contracting process was not the one responsible for the expansion of the UPA 24H, since it occurred more deeply in a previous moment to the beginning of the partnerships with the OSS, however, it also did not contribute to the strengthening of the actuation capacity of the State of Rio de Janeiro, according to the SUS attributions e guidelines. In relation to the assistance aspect, it was concluded that in 19 (nineteen) units there has been an assistance production increase after the management migration to the private sphere and, in total, the average of the production increase was 12%. Finally, with regard to the budget evaluation, it was not possible to compare the expenses with the units, since, although there are data currently available about the expenses with the contracts signed with the OSS, there were not sufficiently available when the units were under the direct management of the SES.
