Resumo

The funding of Brazil's Unified Health System (SUS) has always been a central topic in public debate and on the country's political agenda. Since the inception of the SUS, discussions have focused on ways to improve the allocation of public resources and to increase their amount. The dominant (and correct) diagnosis points to underfunding. This reality poses significant challenges to public health policy makers. The needs of the country continue to grow, while public resources become increasingly scarce. It is widely accepted in the specialized literature that primary care is capable of addressing up to 80% of an individual's lifetime health needs. The Previne Brasil Program (PPB), established in 2019, introduced a new method for financing the costs of primary health care. One of the pillars of the PPB is performance-based remuneration for municipalities, which are responsible for planning and providing basic health care services. The payment-for-performance paradigm, combined with the weighted capitation component of the Program, has led to significant changes in the distribution of resources allocated to primary care among Brazilian municipalities. Urban municipalities, characterized by a high population density and low per capita revenue, have been the most adversely affected. This descriptive study aims to evaluate the implementation of the Program and, consequently, investigate the main factors that have led this specific group of municipalities to experience a relative decrease in resources.
