Resumo

When discussing health care, needs are unlimited and resources, scarce. The current situation and the future perspective for the Brazilian health care system make one search for new ways of improving quality and reducing treatments’ costs, which could lead to increasing and better access to services and information for patients. Porter and Teisberg (2007), also wondering about ways to improve health care, introduced a new look on quality and costs with the concept of value-based health care measured by the value-based equation: quality divided by costs, considering the whole cycle of treatment. That being said, the health care value measurement theme constant to the present study intends to answer the following question: what is the viability of implementing a value-based health care measurement model based in the one proposed by Porter and Teisberg (2007) in Brazil? The purpose herein is to analyze, through the elaboration of a practical model, the viability of this implementation. A quantitative theoretic model was developed as to have practical applicability, therefore creating the aforementioned practical model. As for the health condition chosen as reference for the development of the research, the treatment of cataract at specialized care units for ophthalmology was chosen. The Treatment Value Indicator (IVT) is the result of the division between the Quality Index (QI) and the total Costs for the complete treatment (C). The calculus, performed on a digital spreadsheet, are reconciled, ensuring the integrity of the model’s reasoning, results and scalability. In order to calculate future costs, company budgets were previously elaborated. The inferred results allow us to consider possible to implement a value-based health care measurement model in Brazil. It is important to point out that the best experience as a pilot should validate and improve the model. Apart from aggregating more quality and rationalizing costs, the cooperation between the care units, the administrative and the financial departments generates information that might be useful for health care management in general, although none of it will be advantageous if the participants do not share a firm stand on commitment and trust, given the dense volume and relevance of the information in hand. As a final reminder, the figure that accounts for the individuals who take part in the Brazilian health care system, public or private, reveals how grand the dream of a higher quality and a much costless health care is in the country.
