Cooperação técnica Sul-Sul, capacidades estatais e desenvolvimento socialo caso do projeto de fortalecimento da autoridade sanitária do Haiti

Resumo

Brazil’s official approach to South-South technical cooperation focuses on strengthening state capacities for development, guided by principles such as horizontality, non-conditionality, and the commitment to share successful experiences with other developing countries. Ever since Brazil took military command of MINUSTAH in 2004, Haiti has acquired a central role in its international relations. The earthquake of 2010 marked the enhancement of cooperation between the two countries. Haiti recognizes social rights in its Constitution, including the right to public health. The social action of the State is, however, fragile. In 2010, the "BRA/10/005 - South-South cooperation project to strengthen Haiti's health authority" was entered as the instrument through which Brazil would fulfill the majority of the commitments undertaken in the Memorandum of Understanding executed by Brazil, Cuba and Haiti to "strengthen the public health system and epidemiologic surveillance in Haiti". Among its main activities are: building and renovating health infrastructure; purchasing vehicles and equipment for the public health system; and maintaining the services of the built infrastructures. These activities would not be deemed as the capacity strengthening actions recommended according to the official guidance of Brazilian cooperation. By reviewing literature on State action for social development, State capacities and international cooperation for development; plus research of documents and administrative records available and interviews with relevant actors, this study sought to understand how the design of the BRA/10/005 addresses State capacity-building to strengthen public health in Haiti. The findings reveal that this project represented a relative innovation when compared to technical cooperation projects traditionally presented by Brazil. It strengthened the State’s capacity of territorial coverage and the access to public health, with activities of a collaborative nature that incorporated training and organization of the health care services and of the Haitian health system. Experiences gained through the institutionalization and implementation of the Unified Health System of Brazil were also used as inspiration for the Haitian system. The interviews also unveil "positive side effects" such as the strengthening of communitarian health and of strategies of coordination and management, as well as the creation of a pioneer rehabilitation service for physical disabilities; and even brought forth the possibility of social participation on health policy, which is being discussed by the Haitian government.
