Community participation and decentralization of health services

Resumo

The new Brazilian legislation about the health care system stresses decentralization and community participation. This new model of organizing and producing health care in Brazil needs to be well formulated in order to avoid fragmentation and more deterioration in the provision of services. By considering that the relationship between decentralization and community participation will depend upon social, economic and political setting, it is an utopia to think that community participation will be the panacea for the health problems in Brazil. Although community participation is essential, it is a slow and time consuming process and true community participation will not occur unless communities are given the opportunity to make decisions for themselves and to gain control of health programmes and projects in their area. Because the legislation is not clear about the forms of decentralization and community participation, these topics need to be thoroughly discussed by the consumers of services at community level.
