Resumo

This work brings into focus, in the study of public policies, the issue of the organization of interests in order to understand the dilemmas in the Brazilian process of sanitary reform in the '80s.The study underlines the importance of creating scenarios where social actors may assert privileged and exclusive forms of access to benefits or to public resources. It presents an analysis of thedilemma faced by the health system between a universalist and redistributive project, innovative as to prepositions of equity, and the roll of exclusive and fragmented demands, negotiated by sectors with a greater bargaining power. The autor concludes that the possibility of an enlarged set of ideas on Brazilian citizenship depends on the country's capacity of defining, in the societal plan, the many problems of a fragmented organization of interests.
