Resumo
The paper tries to study the medical practice in its liberal, social welfare and enterprising modalities, going from the historical analysis of its transformation in the capitalism system, particularly in Brazil, till the listening to the patient about this perceptions and expectations on the organizational performance and medical care. Besides the information given as a sample of the three modalities studied, the papel also analyses some categories of the institutional medical speech – coverture extension, free-choice and rationalization of hea1th care - as a comparative instrument of the different situations, conc1uding that the social modalities of medicine have been preserving and even reinforcing a hierarchy of consumption in health care.
