Cultura de segurança do paciente em um hospital acreditadoestudo de caso

Resumo

The present work was carried out in a medium-sized general hospital, located in Minas Gerais, with the objective of analyzing the contributions of accreditation and the evolution of patient safety culture from the perception of frontline health professionals and middle leadership, as well as assessing opportunities for strengthen the patient safety culture. It is an applied, qualitative and descriptive study, developed by the use of case study methodology. The data were obtained using HSOPSC questionnaire of the Agency for Healthcare Research and Quality (AHRQ), a focus group with leaders and institutional documents. The study points out that quality and safety management created the bases for the development of hospital professionals’ safety culture, evidencing that accreditation and the improvement of organizational work processes played an important role. The application of the questionnaire to assess employees’ perception of safety culture revealed that the dimensions with the highest rate of positive responses were: “management support for patient safety”, “organizational learning-continuous improvement” and “supervisor/manager expectations and actions promoting safety”. The dimensions with the lowest positive response rates were: “communication openness”, “handoffs and transitions”, “staffing” and “nonpunitive responses to erros”. These results enabled the identification of opportunities for improvement to guide leadership actions. Moreover, they highlighted the importance of engaging health professionals and their leaders in the strengthening of patient safety culture, seeking safer care.
