Resumo

This work analyzed the level of alignment between the pharmaceutical industry’s research activity and the main population health demands in Brazil and in the world, based on information from global regions organized by income level. New therapies’ active clinical research data was collected from the ClinicalTrials.gov database – one of the most extensive and reliable public clinical trial databases in the world – registered between 2017 and 2021 by the twenty largest global pharmaceutical companies in the year 2020. The research activities were classified by therapeutic area and the respective “burden of disease” measured by DALYs (Disability Adjusted Life Years) in the year 2016. A correlation coefficient was used to measure the main discrepancies between the most important healthcare needs and the concentration of studies in specific therapeutic areas. The correlation between the highest burden of diseases in Brazil and the pharmaceutical trials was considered moderate. Nonetheless, the worldwide correlation was considered low and, the lower the income level of a specific region, the lower the correlation found. In the global view, only high-medium-income and high-income regions were classified with moderate correlations. The results suggest that some therapeutic areas have an excessive concentration of trials when compared to the most important healthcare demands represented by DALYs, in contrast there are areas still lacking innovation. This analysis enlightens the importance of the debate about the pharmaceutical industry’s motivations in the process of research and development, and as well to rethink its social role. This study fosters the need for discussions between public and private sectors looking towards more alignment of interests and, consequently, the sustainability of the pharmaceutical market. Public authorities should propose public policies to facilitate the investment in clinical research related to health priorities, and pharmaceutical companies should apply strategies to continuously balance their launches’ pipelines to attend more effectively high priority public health demands.
